Your initial visit 

Before you are scheduled for your first consult, you will receive an email with an invitation to register with  the CHARM Patient Portal.  Once your account is registered, you can upload and share documents, communicate with me via secure messaging,  view your treatment plans, labs results  and all documents I shared with you and also, request appointments. 

You can reach the Patient Portal via the button below or through the Patient Portal tab on the website. Remember, that you need to be registered to log into the portal.

List of documents requested for your first visit

Please submit all the requested documents no later than 48 hours before your scheduled appointment.  

1. New Patient Details 
2. New Adult Intake Questionnaire (or New Pediatric Questionnaire) 
3. Blood/lab work results in the last 6 months (if available)
4. Imaging reports (X-ray, MRI, ultrasounds, CT scans) (if available)
5. Surgical operative reports (if available)

All these documents can be uploaded on the Patient Portal and shared with me (Note: if you forget to share them, I will not be able to see them).
