Persa Functional Medicine is a self-pay facility and does not have contracts with any medical  insurance providers. Payments are required at the time of service, and can be made with credit cards only (Visa, MC, Discover, AmEx). I do not accept cash or checks.

I will provide you with a traditional “superbill” with the appropriate billing codes needed for insurance filing, but you are responsible for submitting insurance papers for reimbursement yourself. Your insurance provider would typically consider me an “out-of-network provider,” and you may be entitled to coverage through your insurance plan under that category. I would recommend checking with your insurance provider for details. 

When you request your appointment online, your credit card will be charged for $1.00. The full amount for the visit will be due at the time of the service. 

When scheduling your appointment, please pick the type of visit that best suits your needs. For example, a 15-minute appointment would be appropriate for a brief consultation; for more in-depth care, a longer appointment would be necessary. 

For Saturday appointments, holiday appointments or after-hours "emergency" appointments, there will be an extra $100 charge added to the regular appointment fee, i.e. if the regular fee for 15 minutes is $99, the charge for a Saturday or holiday appointment lasting 15 minutes would be $199. All these appointments outside the regular schedule will be approved only if time allows. You will need to send me an email to request this type of appointment.

The following fees apply to in-person, over-the-phone, and video consultations. 

  • 90-Minute New Patient Initial Consultation– $439
  • 75-Minute Consultation– $399
  • 60-Minute Consultation– $359
  • 45-Minute Consultation– $269
  • 30-Minute Consultation– $189
  • 15-Minute Consultation– $99

The minimum amount of time for any appointment is 15 minutes!

All the above fees are for the doctor’s consultations only. They do not include any labs, imaging, supplements, or medications. The average price for lab work is between $800- $1000, or more depending on the complexity of the case.

Cancellation Policy

We all know that emergencies can happen, and I am here to help! I am asking that you, please be kind, and provide at least 48 hours notice for all cancelled appointments. I assure you my preparation for your visit will start well before this time frame. Also, when you provide this notice, I can accommodate other patients on the waiting list.  

Remember, that any “in-person” appointment can be turned into a phone or telemedicine appointment if needed.

A $100-fee will be charged for late cancellations inside of 48 hours, and the full appointment fee will be charged for cancellations with less than 24-hours notice as well as for no-show visits. Regretfully, two consecutive cancellations (without proper notice), or “no shows,” will result in the patient being discharged from my practice.
