ChARM scheduling system will send appointment confirmation/reminders to the email address you provided.

When available, the lab test results will be shared with you via patient portal- once you log into your patient account you can find them under “documents.” Lab results will be reviewed in detail with you at the time of your appointment.

Medication refills are typically addressed during your office appointments. If refills are needed in between appointments, please have your pharmacy fax a refill request to my office. My office fax number is 833-260-9330.

Your refill request will be completed within 48 hours (or 2 business days). Please, be sure to request the refills 2-3 weeks before they run out. 

You must be an active patient, seen in the past 12 months for an appointment and have the required lab work up to date, to receive ongoing refills. Patients receiving testosterone therapy need to be seen every 6 months to receive refills.
